Role Summary

Utilizing telephonic and virtual outreach methods, the Evernorth Home Based Care, Social Worker is responsible for coordinating with patients and key clinical partners to aid in the delivery of cost-effective, high quality health care. The social worker will support Evernorth Health Services programs aimed at meeting the complex needs of our clients including advanced care planning, psychosocial support, and complex care coordination. Social workers will attend in-person trainings/meetings on an as-needed basis.

For this position, we anticipate offering an annual salary of 56,600 - 94,400 USD / yearly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
