Division/Department/Program: DR3AM'RS Clubhouse (DREAMER'S)
DR3AM'RS Clubhouse Program is designed to provide a comprehensive and unique set of services for children and families coping with the isolation, stigma and other challenges of substance use and co-occurring (substance abuse and mental health) disorders and related behavioral health disorders.
The Clubhouse Case Manager assists clubhouse members in learning life skills that will enhance their quality of life and will operate as support to the Clubhouse Director and Clubhouse Staff.
Duties & Responsibilities:
Works with the manager to help the clubhouse members and their families.
Provides on-going support and case management expertise to clubhouse members
Works with the adolescents, their family and counselors in identifying individual needs for each child, assisting with conflict resolution and creating action plans to assist family and child with the completion of established goals.
Manage a caseload of youth, ensuring timely follow-up on services, documentation, and communication.
Empowers and encourages kids to make use of all identified resources at the Clubhouse.
Attend all training opportunities as provided.
Motivate youth to stay in school and overcome drug problems, decrease DJJ involvement, improve family involvement and relationships and increase family social function.
Maintain accurate and detailed client records, including progress notes, service delivery documentation, and reports as required by agency guidelines.
Assist with transporting members using 12 passenger vans and minivans.
Assist with planning and ensuring proper nutrition for clubhouse members while participating in regular activities.
Other duties as assigned.
Minimum Qualifications:
Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.
Preferred Qualifications:
Bilingual (English & Spanish speaking)
Must be patient and flexible.
Must have the ability to listen without judgment.
Flexible time includes Saturday activities.
Must be computer literate on Microsoft Office, Excel, Access and Windows applications.
Must have excellent human relations skills, having dealt with a variety of customers and employees
Requirements/Competencies:
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities
Note: Some positions may require a valid driver's license.
